This page explains how applicants should name documents related to 6. Other Documentary Evidence of Achievements (if applicable) when uploading them to their online application page.

in the application guidelines and "How to apply" in addition to the following information.
The List of Academic Achievements or Awards
Please use the “Additional Information” file that you can download from your online application page to list the pieces of evidence, and upload your documentary evidence. You can also list some additional items without documentary evidence if you wish to. The PEAK Undergraduate Admissions Office reserves the right to request additional documents.
Please upload the list in Excel or PDF format. The Numbers format cannot be accepted.

If you submit multiple pieces of evidence, please use a different number for each piece of evidence (_1, _2, _3).
Example:

If the document is in other languages (not English)
In addition to submitting the translated (English) document using the names mentioned above, you need to submit the original (not English) document using the following name.
6_Other Documentary Evidence_local_1
For example, “6_Other Documentary Evidence_1” should be the English translation of “6_Other Documentary Evidence_local_1
